Revised 8-15-2005
New Patron
Application Form
Any resident in the Aberdeen District Library boundaries is entitled to a borrower’s card unless their privileges have been revoked. Cards may also be issued to patrons of partner libraries (American Falls, Snake River, Marshal Public Library and Blackfoot) and approved non-residents. Application form and internet user agreement must be filled out for every patron requesting a library card.
Child applications and internet user agreement forms are to be filled out at the library by a parent or guardian unless we have a valid parent or guardian signature on file. Patrons need to be six years old to be eligible for a library card. Juveniles under the age of 18 must have a parent or guardian signed application form. Until the application is completed a person cannot check out materials or use the computers.
The first card will be issued for the patron to use at no cost. The patron whose name is on the card is responsible for all materials checked out on their card. Parents are responsible for fines or materials checked out to minor children. No one may use another patron’s card.
There are two types of cards available. A single patron card is for use by one person and has a limit of five items. A family card is for use by an entire family and has a limit of 25 items with a maximum of five audio-visual materials. A family must choose to do a family card or have single patron cards. There is a $5.00 charge for replacement cards.
Check-out of Materials
Patron’s library card is requested prior to checking out items. If they have forgotten their card, their number is looked up on the computer and they are reminded they need to bring it with them each time. If they have lost their card they will need to purchase a replacement.
The barcode for each item is scanned into the computer and a date due slip is printed for the patron.
If an item is found without a barcode it needs to be sent to cataloging to be properly processed.
Reference Books
Reference books and special collections will not be available for check out. Patrons will be given the opportunity to copy pages (within reason) at the library at no charge. Some reference books are marked and can be checked out.

Check-in Materials
The barcode for each returned item is scanned into the computer. Each retuned item is to be checked and noted for damage and sent for repair if necessary. Items are then returned to the shelf for check out.
Reserves
A patron may reserve library materials that are not unavailable when they request it.
In Horizon enter the patron’s card number in the check out window. Press F2 and search for the item. When the record for the item comes up click on the request menu and select make request.
When the item is returned the computer will give notice that the item has been requested. Print out the hold slip. Attach the slip to the book, phone the patron to let them know it will be held for one week before being retuned to circulation.
